The timezone appears to be off as the "Last upgrade" shows as "01/01/70 6:00p". In Call Manager, both the Registration and IP Address display "Unknown". As Roger said, we have limited data on your setup, but if it is CUCM I'd check that the phone is built as the correct model and try clearing the security settings (ITL/CTL) on the device.
